This is the temporary simple fix for 3.2, we need more changes in this
area.
1. do_signal_stop() assumes that the running untraced thread in the
stopped thread group is not possible. This was our goal but it is
not yet achieved: a stopped-but-resumed tracee can clone the running
thread which can initiate another group-stop.
Remove WARN_ON_ONCE(!current->ptrace).
2. A new thread always starts with ->jobctl = 0. If it is auto-attached
and this group is stopped, __ptrace_unlink() sets JOBCTL_STOP_PENDING
but JOBCTL_STOP_SIGMASK part is zero, this triggers WANR_ON(!signr)
in do_jobctl_trap() if another debugger attaches.
Change __ptrace_unlink() to set the artificial SIGSTOP for report.
Alternatively we could change ptrace_init_task() to copy signr from
current, but this means we can copy it for no reason and hide the
possible similar problems.

It fails because ->real_parent sees its child in EXIT_DEAD state
while the tracer is going to change the state back to EXIT_ZOMBIE
in wait_task_zombie().
The offending commit is 823b018e which moved the EXIT_DEAD check,
but in fact we should not blame it. The original code was not
correct as well because it didn't take ptrace_reparented() into
account and because we can't really trust ->ptrace.
This patch adds the additional check to close this particular
race but it doesn't solve the whole problem. We simply can't
rely on ->ptrace in this case, it can be cleared if the tracer
is multithreaded by the exiting ->parent.
I think we should kill EXIT_DEAD altogether, we should always
remove the soon-to-be-reaped child from ->children or at least
we should never do the DEAD->ZOMBIE transition. But this is too
complex for 3.2.

The call to "schedule_work()" in rtc_initialize_alarm() happens too
early, and can cause oopses at bootup
Neil Brown explains why we do it:
"If you set an alarm in the future, then shutdown and boot again after
that time, then you will end up with a timer_queue node which is in
the past.
When this happens the queue gets stuck. That entry-in-the-past won't
get removed until and interrupt happens and an interrupt won't happen
because the RTC only triggers an interrupt when the alarm is "now".
So you'll find that e.g. "hwclock" will always tell you that
'select' timed out.
So we force the interrupt work to happen at the start just in case."
and has a patch that convert it to do things in-process rather than with
the worker thread, but right now it's too late to play around with this,
so we just revert the patch that caused problems for now.

Turned out the ntlmv2 (default security authentication)
upgrade was harder to test than expected, and we ran
out of time to test against Apple and a few other servers
that we wanted to. Delay upgrade of default security
from ntlm to ntlmv2 (on mount) to 3.3. Still works
fine to specify it explicitly via "sec=ntlmv2" so this
should be fine.

The current check looks to see if the RFC1002 length is larger than
CIFSMaxBufSize, and fails if it is. The buffer is actually larger than
that by MAX_CIFS_HDR_SIZE.
This bug has been around for a long time, but the fact that we used to
cap the clients MaxBufferSize at the same level as the server tended
to paper over it. Commit c974befa changed that however and caused this
bug to bite in more cases.

It causes failures on Toshiba laptops - instead of disabling the alarm,
it actually seems to enable it on the affected laptops, resulting in
(for example) the laptop powering on automatically five minutes after
shutdown.
There's a patch for it that appears to work for at least some people,
but it's too late to play around with this, so revert for now and try
again in the next merge window.

Commit 1e39f384bb ("evm: fix build problems") makes the stub version
of security_old_inode_init_security() return 0 when CONFIG_SECURITY is
not set.
But that makes callers such as reiserfs_security_init() assume that
security_old_inode_init_security() has set name, value, and len
arguments properly - but security_old_inode_init_security() left them
uninitialized which then results in interesting failures.
Revert security_old_inode_init_security() to the old behavior of
returning EOPNOTSUPP since both callers (reiserfs and ocfs2) handle this
just fine.
[ Also fixed the S_PRIVATE(inode) case of the actual non-stub
security_old_inode_init_security() function to return EOPNOTSUPP
for the same reason, as pointed out by Mimi Zohar.
It got incorrectly changed to match the new function in commit
fb88c2b6cb: "evm: fix security/security_old_init_security return

If 'iw connect' command is fired when driver is already busy in
serving 'iw scan' command, ssid specific scan operation for connect
is skipped. In this case cmd wait queue handler gets called with no
command in queue (i.e. adapter->cmd_queued = NULL).
This patch adds a NULL check in mwifiex_wait_queue_complete()
routine to fix crash observed during simultaneous scan and assoc
operations.

This is because fec.h uses CONFIG_SOC_IMX28 to determine the register
layout of the core which makes sense since the MX23 does not have a fec.
However, Kconfig uses the broader ARCH_MXS symbol and this way even
makes the fec-driver default for MX23. Adapt Kconfig to use the more
precise SOC_IMX28 as well.

The current gspca core code has a regression where it no longer properly
falls back to lower alt settings when there is not enough bandwidth.
This causes many iso based usb-1 cameras to not work when plugged into a
usb2 hub or a sandybridge chipset motherboard!
This patch fixes this.

It was found (by Sasha) that if you use a futex located in the gate
area we get stuck in an uninterruptible infinite loop, much like the
ZERO_PAGE issue.
While looking at this problem, PeterZ realized you'll get into similar
trouble when hitting any install_special_pages() mapping. And are there
still drivers setting up their own special mmaps without page->mapping,
and without special VM or pte flags to make get_user_pages fail?
In most cases, if page->mapping is NULL, we do not need to retry at all:
Linus points out that even /proc/sys/vm/drop_caches poses no problem,
because it ends up using remove_mapping(), which takes care not to
interfere when the page reference count is raised.
But there is still one case which does need a retry: if memory pressure
called shmem_writepage in between get_user_pages_fast dropping page
table lock and our acquiring page lock, then the page gets switched from
filecache to swapcache (and ->mapping set to NULL) whatever the refcount.
Fault it back in to get the page->mapping needed for key->shared.inode.

and so is the division, which should have been a signed division.

The new iso bandwidth calculation code accidentally has broken support
for bulk mode cameras. This has broken the following drivers:

Ceph attempts to use the dcache to satisfy negative lookups and readdir
when the entire directory contents are in cache. Disable this behavior
until lingering bugs in this code are shaken out; we'll re-enable these
hooks once things are fully stable.

Commit 5e081591 "block: warn if tag is greater than real_max_depth"
cleaned up blk_queue_end_tag() to warn when the tag is truly invalid
(greater than real_max_depth). However, it changed behavior in the tag <
max_depth case to not end the request. Leading to triggering of
BUG_ON(blk_queued_rq(rq)) in the request completion path:

In order to allow blk_queue_resize_tags() to shrink the tag space
blk_queue_end_tag() must always complete tags with a value less than
real_max_depth regardless of the current max_depth. The comment about
"handling the shrink case" seems to be what prompted changes in this
space, so remove it and BUG on all invalid tags (made even simpler by
Matthew's suggestion to use an unsigned compare).

Following is happened when CONFIG_CPU_FREQ_S3C24XX_DEBUGFS
is selected without building of s3c2410-iotiming.c file:
arch/arm/mach-s3c2440/built-in.o:(.data+0x38c): undefined reference to `s3c2410_iotiming_debugfs
Basically, the CONFIG_S3C2410_IOTIMING is not selected for
MACH_MINI2440. Because the s3c2410-iotiming.c is not ever
compiled and enabling CONFIG_CPU_FREQ_S3C24XX_DEBUGFS option
caused undefined reference to s3c2410_iotiming_debugfs()
defined in that file. The s3c2410_iotiming_debugfs defined
as NULL for this case.

The previous patch breaks reset watchdog behaviour on the older hardware.
It is therefor better to make sure that the behaviour for older hardware (<=ICH5 or
6300ESB) is preserved and that the behaviour for newer hardware is changed.
We therefor use the iTCO_version to see if we need the clearing of the SMI_TCO_EN
bit in the SMI_EN register.
So the new behaviour becomes:
turn_SMI_watchdog_clear_off=0 -> Do not turn off SMI clearing watchdog.
turn_SMI_watchdog_clear_off=1 -> Turn off SMI clearing watchdog when iTCO_version=1
(ICHO till ICH5 + 6300ESB only)
turn_SMI_watchdog_clear_off=2 -> Turn off SMI clearing watchdog.

Bruce Fields notes that commit 778fc546f7 ("locks: fix tracking of
inprogress lease breaks") introduced a possible error pointer
dereference on failure to allocate memory. locks_conflict() will
dereference the passed-in new lease lock structure that may be an error pointer.
This means an open (without O_NONBLOCK set) on a file with a lease
applied (generally only done when Samba or nfsd (with v4) is running)
could crash if a kmalloc() fails.
So instead of playing games with IS_ERROR() all over the place, just
check the allocation failure early. That makes the code more
straightforward, and avoids this possible bad pointer dereference.

The AMBA ID table is marked as __initdata, yet it is referenced by the
driver struct which is not. This causes a (somewhat unhelpful) section
mismatch warning:

Currently kvmppc_start_thread() tries to wake other SMT threads via
xics_wake_cpu(). Unfortunately xics_wake_cpu only exists when
CONFIG_SMP=Y so when compiling with CONFIG_SMP=N we get:

Unlike all of the other cpuid bits, the TSC deadline timer bit is set
unconditionally, regardless of what userspace wants.
This is broken in several ways:
- if userspace doesn't use KVM_CREATE_IRQCHIP, and doesn't emulate the TSC
deadline timer feature, a guest that uses the feature will break
- live migration to older host kernels that don't support the TSC deadline
timer will cause the feature to be pulled from under the guest's feet;
breaking it
- guests that are broken wrt the feature will fail.
Fix by not enabling the feature automatically; instead report it to userspace.
Because the feature depends on KVM_CREATE_IRQCHIP, which we cannot guarantee

Only allow KVM device assignment to attach to devices which:
- Are not bridges
- Have BAR resources (assume others are special devices)
- The user has permissions to use
Assigning a bridge is a configuration error, it's not supported, and
typically doesn't result in the behavior the user is expecting anyway.
Devices without BAR resources are typically chipset components that
also don't have host drivers. We don't want users to hold such devices
captive or cause system problems by fencing them off into an iommu
domain. We determine "permission to use" by testing whether the user
has access to the PCI sysfs resource files. By default a normal user
will not have access to these files, so it provides a good indication
that an administration agent has granted the user access to the device.

Prevent this by checking the irqchip mode before starting a timer. We
can't deny creating the PIT if the irqchips aren't set up yet as
current user land expects this order to work.

Since Linux 2.6.36 the writeback code has introduces various measures for
live lock prevention during sync(). Unfortunately some of these are
actively harmful for the XFS model, where the inode gets marked dirty for
metadata from the data I/O handler.
The older_than_this checks that are now more strictly enforced since
writeback: avoid livelocking WB_SYNC_ALL writeback
by only calling into __writeback_inodes_sb and thus only sampling the
current cut off time once. But on a slow enough devices the previous
asynchronous sync pass might not have fully completed yet, and thus XFS
might mark metadata dirty only after that sampling of the cut off time for
the blocking pass already happened. I have not myself reproduced this
myself on a real system, but by introducing artificial delay into the
XFS I/O completion workqueues it can be reproduced easily.
Fix this by iterating over all XFS inodes in ->sync_fs and log all that
are dirty. This might log inode that only got redirtied after the
previous pass, but given how cheap delayed logging of inodes is it
isn't a major concern for performance.

If the writeback code writes back an inode because it has expired we currently
use the non-blockin ->write_inode path. This means any inode that is pinned
is skipped. With delayed logging and a workload that has very little log
traffic otherwise it is very likely that an inode that gets constantly
written to is always pinned, and thus we keep refusing to write it. The VM
writeback code at that point redirties it and doesn't try to write it again
for another 30 seconds. This means under certain scenarious time based
metadata writeback never happens.
Fix this by calling into xfs_log_inode for kupdate in addition to data
integrity syncs, and thus transfer the inode to the log ASAP.

Activation conditions for a workaround should not be encoded in the
workaround's direct dependencies if this makes otherwise reasonable
configuration choices impossible.
This patches uses the SMP/UP patching facilities instead to compile
out the workaround if the configuration means that it is definitely
not needed.
This means that configs for buggy silicon can simply select
ARM_ERRATA_751472, without preventing a UP kernel from being built
or duplicatiing knowledge about when to activate the workaround.
This seems the correct way to do things, because the erratum is a
property of the silicon, irrespective of what the kernel config
happens to be.

Activation conditions for a workaround should not be encoded in the

The workaround for erratum 720789 only affects a code path which is
not active in UP kernels; hence it should be safe to turn on in UP
kernels, without penalty.
This patch simply removes the extra dependency on SMP from Kconfig.
This means that configs for buggy silicon can simply select
ARM_ERRATA_720789, without preventing a UP kernel from being built
or duplicatiing knowledge about when to activate the workaround.

"! --connbytes 23:42" should match if the packet/byte count is not in range.
As there is no explict "invert match" toggle in the match structure,
userspace swaps the from and to arguments
(i.e., as if "--connbytes 42:23" were given).
However, "what <= 23 && what >= 42" will always be false.
Change things so we use "||" in case "from" is larger than "to".
This change may look like it breaks backwards compatibility when "to" is 0.
However, older iptables binaries will refuse "connbytes 42:0",
and current releases treat it to mean "! --connbytes 0:42",
so we should be fine.

This closes races where btrfs is calling d_instantiate too soon during
inode creation. All of the callers of btrfs_add_nondir are updated to
instantiate after the inode is fully setup in memory.

This change fixes a linking problem, which happens if oprofile
is selected to be compiled as built-in:
`oprofile_arch_exit' referenced in section `.init.text' of
arch/arm/oprofile/built-in.o: defined in discarded section
`.exit.text' of arch/arm/oprofile/built-in.o
The problem is appeared after commit 87121ca504, which
introduced oprofile_arch_exit() calls from __init function. Note
that the aforementioned commit has been backported to stable
branches, and the problem is known to be reproduced at least
with 3.0.13 and 3.1.5 kernels.

commit d0a4bb4927 introduced a
regression which is annoying but fairly harmless.
When writing to an array that is undergoing recovery (a spare
in being integrated into the array), writing to the array will
set bits in the bitmap, but they will not be cleared when the
write completes.
For bits covering areas that have not been recovered yet this is not a
problem as the recovery will clear the bits. However bits set in
already-recovered region will stay set and never be cleared.
This doesn't risk data integrity. The only negatives are:
- next time there is a crash, more resyncing than necessary will
be done.
- the bitmap doesn't look clean, which is confusing.
While an array is recovering we don't want to update the
'events_cleared' setting in the bitmap but we do still want to clear
bits that have very recently been set - providing they were written to
the recovering device.
So split those two needs - which previously both depended on 'success'
and always clear the bit of the write went to all devices.

Before performing a recovery we try to remove any spares that
might not be working, then add any that might have become relevant.
Currently we abort on the first spare that cannot be added.
This is a false optimisation.
It is conceivable that - depending on rules in the personality - a
subsequent spare might be accepted.
Also the loop does other things like count the available spares and
reset the 'recovery_offset' value.
If we abort early these might not happen properly.
So remove the early abort.
In particular if you have an array what is undergoing recovery and
which has extra spares, then the recovery may not restart after as
reboot as the could of 'spares' might end up as zero.

While reshaping a degraded array (as when reshaping a RAID0 by first
converting it to a degraded RAID4) we currently get confused about
which devices are in_sync. In most cases we get it right, but in the
region that is being reshaped we need to treat non-failed devices as
in-sync when we have the data but haven't actually written it out yet.

broke hot-add to a linear array.
After that commit, metadata if not written to devices until they
have been fully integrated into the array as determined by
saved_raid_disk. That patch arranged to clear that field after
a recovery completed.
However for linear arrays, there is no recovery - the integration is
instantaneous. So we need to explicitly clear the saved_raid_disk
field.

This silently was working for many years and stopped working on
Niagara-T3 machines.
We need to set the MSIQ to VALID before we can set it's state to IDLE.
On Niagara-T3, setting the state to IDLE first was causing HV_EINVAL
errors. The hypervisor documentation says, rather ambiguously, that
the MSIQ must be "initialized" before one can set the state.
I previously understood this to mean merely that a successful setconf()
operation has been performed on the MSIQ, which we have done at this
point. But it seems to also mean that it has been set VALID too.

Currently, the *_global_[un]lock_online() routines are not at all synchronized
with CPU hotplug. Soft-lockups detected as a consequence of this race was
reported earlier at https://lkml.org/lkml/2011/8/24/185. (Thanks to Cong Meng
for finding out that the root-cause of this issue is the race condition
between br_write_[un]lock() and CPU hotplug, which results in the lock states
getting messed up).
Fixing this race by just adding {get,put}_online_cpus() at appropriate places
in *_global_[un]lock_online() is not a good option, because, then suddenly
br_write_[un]lock() would become blocking, whereas they have been kept as
non-blocking all this time, and we would want to keep them that way.
So, overall, we want to ensure 3 things:
1. br_write_lock() and br_write_unlock() must remain as non-blocking.
2. The corresponding lock and unlock of the per-cpu spinlocks must not happen
for different sets of CPUs.
3. Either prevent any new CPU online operation in between this lock-unlock, or
ensure that the newly onlined CPU does not proceed with its corresponding
per-cpu spinlock unlocked.
To achieve all this:
(a) We introduce a new spinlock that is taken by the *_global_lock_online()
routine and released by the *_global_unlock_online() routine.
(b) We register a callback for CPU hotplug notifications, and this callback
takes the same spinlock as above.
(c) We maintain a bitmap which is close to the cpu_online_mask, and once it is
initialized in the lock_init() code, all future updates to it are done in
the callback, under the above spinlock.
(d) The above bitmap is used (instead of cpu_online_mask) while locking and
unlocking the per-cpu locks.
The callback takes the spinlock upon the CPU_UP_PREPARE event. So, if the
br_write_lock-unlock sequence is in progress, the callback keeps spinning,
thus preventing the CPU online operation till the lock-unlock sequence is
complete. This takes care of requirement (3).
The bitmap that we maintain remains unmodified throughout the lock-unlock
sequence, since all updates to it are managed by the callback, which takes
the same spinlock as the one taken by the lock code and released only by the
unlock routine. Combining this with (d) above, satisfies requirement (2).
Overall, since we use a spinlock (mentioned in (a)) to prevent CPU hotplug
operations from racing with br_write_lock-unlock, requirement (1) is also
taken care of.
By the way, it is to be noted that a CPU offline operation can actually run
in parallel with our lock-unlock sequence, because our callback doesn't react
to notifications earlier than CPU_DEAD (in order to maintain our bitmap
properly). And this means, since we use our own bitmap (which is stale, on
purpose) during the lock-unlock sequence, we could end up unlocking the
per-cpu lock of an offline CPU (because we had locked it earlier, when the
CPU was online), in order to satisfy requirement (2). But this is harmless,
though it looks a bit awkward.

this.
This driver depends upon CONFIG_OF_IRQ but not all CONFIG_OF platforms
support that infrastructure, in particular Sparc does not so the
build fails.
Please push a patch like the following to Linus so that this code only
gets built where it actually should.

flow_cach_flush() might sleep but can be called from
atomic context via the xfrm garbage collector. So add
a flow_cache_flush_deferred() function and use this if
the xfrm garbage colector is invoked from within the
packet path.

removed IP route cache garbage collector a bit too soon, as this gc was
responsible for expired routes cleanup, releasing their neighbour
reference.
As pointed out by Robert Gladewitz, recent kernels can fill and exhaust
their neighbour cache.
Reintroduce the garbage collection, since we'll have to wait our
neighbour lookups become refcount-less to not depend on this stuff.

makes IP-Config wait for carrier on at least one network device.
Before waiting (predefined value 120s), check that at least one device
was successfully brought up. Otherwise (e.g. buggy bootloader
which does not set the MAC address) there is no point in waiting
for carrier.

When checking whether a DATA chunk fits into the estimated rwnd a
full sizeof(struct sk_buff) is added to the needed chunk size. This
quickly exhausts the available rwnd space and leads to packets being
sent which are much below the PMTU limit. This can lead to much worse
performance.
The reason for this behaviour was to avoid putting too much memory
pressure on the receiver. The concept is not completely irational
because a Linux receiver does in fact clone an skb for each DATA chunk
delivered. However, Linux also reserves half the available socket
buffer space for data structures therefore usage of it is already
accounted for.
When proposing to change this the last time it was noted that this
behaviour was introduced to solve a performance issue caused by rwnd
overusage in combination with small DATA chunks.
Trying to reproduce this I found that with the sk_buff overhead removed,
the performance would improve significantly unless socket buffer limits
are increased.
The following numbers have been gathered using a patched iperf
supporting SCTP over a live 1 Gbit ethernet network. The -l option
was used to limit DATA chunk sizes. The numbers listed are based on
the average of 3 test runs each. Default values have been used for
sk_(r|w)mem.

binary_sysctl() calls sysctl_getname() which allocates from names_cache
slab usin __getname()
The matching function to free the name is __putname(), and not putname()
which should be used only to match getname() allocations.
This is because when auditing is enabled, putname() calls audit_putname
*instead* (not in addition) to __putname(). Then, if a syscall is in
progress, audit_putname does not release the name - instead, it expects
the name to get released when the syscall completes, but that will happen
only if audit_getname() was called previously, i.e. if the name was
allocated with getname() rather than the naked __getname(). So,
__getname() followed by putname() ends up leaking memory.

An integer overflow will happen on 64bit archs if task's sum of rss,
swapents and nr_ptes exceeds (2^31)/1000 value. This was introduced by
commit
f755a04 oom: use pte pages in OOM score
where the oom score computation was divided into several steps and it's no
longer computed as one expression in unsigned long(rss, swapents, nr_pte
are unsigned long), where the result value assigned to points(int) is in
range(1..1000). So there could be an int overflow while computing
176 points *= 1000;
and points may have negative value. Meaning the oom score for a mem hog task
will be one.

Out of memory: Kill process 3366 (oom01) score 1 or sacrifice child
Here the oom1 process consumes more than 24303939(rss)*4096~=92GB physical
memory, but it's oom score is one.
In this situation the mem hog task is skipped and oom killer kills another and
most probably innocent task with oom score greater than one.
The points variable should be of type long instead of int to prevent the
int overflow.

There is a potential integer overflow in nilfs_ioctl_clean_segments().
When a large argv[n].v_nmembs is passed from the userspace, the subsequent
call to vmalloc() will allocate a buffer smaller than expected, which
leads to out-of-bound access in nilfs_ioctl_move_blocks() and
lfs_clean_segments().
The following check does not prevent the overflow because nsegs is also
controlled by the userspace and could be very large.

This patch clamps argv[n].v_nmembs to UINT_MAX / argv[n].v_size, and
returns -EINVAL when overflow.

Kernels where MAX_NUMNODES > BITS_PER_LONG may temporarily see an empty
nodemask in a tsk's mempolicy if its previous nodemask is remapped onto a
new set of allowed cpuset nodes where the two nodemasks, as a result of
the remap, are now disjoint.
c0ff7453bb ("cpuset,mm: fix no node to alloc memory when changing
cpuset's mems") adds get_mems_allowed() to prevent the set of allowed
nodes from changing for a thread. This causes any update to a set of
allowed nodes to stall until put_mems_allowed() is called.
This stall is unncessary, however, if at least one node remains unchanged
in the update to the set of allowed nodes. This was addressed by
89e8a244b9 ("cpusets: avoid looping when storing to mems_allowed if one
node remains set"), but it's still possible that an empty nodemask may be
read from a mempolicy because the old nodemask may be remapped to the new
nodemask during rebind. To prevent this, only avoid the stall if there is
no mempolicy for the thread being changed.
This is a temporary solution until all reads from mempolicy nodemasks can
be guaranteed to not be empty without the get_mems_allowed()
synchronization.
Also moves the check for nodemask intersection inside task_lock() so that
tsk->mems_allowed cannot change. This ensures that nothing can set this
tsk's mems_allowed out from under us and also protects tsk->mempolicy.

irq_set_chained_handler sets 'desc->handle_irq'.
However this irq is called by handle_nested_irq from handle_twl4030_pih,
and that uses action->thread_fn.
So the handled set with irq_set_chained_handler is never called.
So change to use request_threaded_irq instead - that sets the correct field.

As the interrupt source is only cleared by the threaded interrupt
service routine, we need to make the base interrupt IRQF_ONESHOT.
Without this, the first interrupt from the TWL4030 cause the CPU to
enter an infinite loop trying to handle to interrupt but never
clearing it.

On multi-core systems, setting of the key before every caclculation,
causes invalid HMAC calculation for other tfm users, because internal
state (ipad, opad) can be invalid before set key call returns.
It needs to be set only once during initialization.

The former implementation adds a fix gpiochip label string
to the framework. This is confusing because orion_gpio_init
is called more than once and this ends up in different gpiochips
with the same label.
This patch adds the already present orion_gpio_chip_count to the
label string to make it unique in the system.

When SDIO runtime PM was originally introduced, we immediately faced
two regressions with two different chipsets, and in response decided
not to enable it by default.
With the recent work on the 8686 we hoped we found all the gotchas,
so 08da834 did make sense (at least experimentally).
Unfortunately we now see that some setups out there still refuse to
work when SDIO runtime PM is enabled by default (see
http://www.spinics.net/lists/linux-mmc/msg11161.html), and obviously
we can't live with these kind of regressions.
This reverts commit 08da834a24.

Commit 8ffd3208 voids the previous patches f6778aab and 810c0719 for
limiting the autoclose value. If userspace passes in -1 on 32-bit
platform, the overflow check didn't work and autoclose would be set
to 0xffffffff.
This patch defines a max_autoclose (in seconds) for limiting the value
and exposes it through sysctl, with the following intentions.
1) Avoid overflowing autoclose * HZ.
2) Keep the default autoclose bound consistent across 32- and 64-bit
platforms (INT_MAX / HZ in this patch).
3) Keep the autoclose value consistent between setsockopt() and
getsockopt() calls.

When printing the code bytes in show_registers(), the markers around the
byte at the fault address could make the printk() format string look
like a valid log level and facility code. This would prevent this byte
from being printed and result in a spurious newline:

Received non stream protocol packets were calling llc_cmsg_rcv that used a
skb after that skb was released by sk_eat_skb. This caused received STP

x86 jump instruction size is 2 or 5 bytes (near/long jump), not 2 or 6
bytes.
In case a conditional jump is followed by a long jump, conditional jump

Since we configure all the queues as CHAINABLE, we need to update the
byte count for all the queues, not only the AGGREGATABLE ones.
Not doing so can confuse the SCD and make the fw assert.

private_data_len is defined as a u8. If the user specifies a large
private_data size (> 220 bytes), we will calculate a total length that
exceeds 255, resulting in private_data_len wrapping back to 0. This
can lead to overwriting random kernel memory. Avoid this by verifying
that the resulting size fits into a u8.

If oprofilefs_ulong_from_user() is called with count equals
zero, *val remains unchanged. Depending on the implementation it
might be uninitialized.
Change oprofilefs_ulong_from_user()'s interface to return count
on success. Thus, we are able to return early if count equals
zero which avoids using *val uninitialized. Fixing all users of
oprofilefs_ulong_ from_user().
This follows write syscall implementation when count is zero:
"If count is zero ... [and if] no errors are detected, 0 will be
returned without causing any other effect." (man 2 write)

We should not change gadget driver's descriptors just
because we think it's right to do so.
There are several of reasons which would support this
statement but it suffices to say that this was probably
never tested because it updates bmAttributes without
asking the driver if it's ok to do so.
This means that e.g. on UASP gadget it would enable
stream support even for the command endpoint which must
not have stream support enabled.
In fact, this change is fixing the bug introduced by
commit a59d6b9 (usb: gadget: add streams support to
the gadget framework) which was caught when testing
UASP gadget with dwc3 driver.

When the last RFCOMM data channel is closed, a timer is normally set
up to disconnect the control channel at a later time. If the control
channel disconnect command is sent with the timer pending, the timer
needs to be cancelled.
If the timer is not cancelled in this situation, the reference
counting logic for the RFCOMM session does not work correctly when the
remote device closes the L2CAP connection. The session is freed at
the wrong time, leading to a kernel panic.

When configuring an ERTM or streaming mode connection, remote devices
are expected to send an RFC option in a successful config response. A
misbehaving remote device might not send an RFC option, and the L2CAP
code should not access uninitialized data in this case.

I noticed that hotplug of one setup does not work with recent change in
pci tree.
After checking the bridge conf setup, I noticed that the bridges get
assigned but do not get enabled.
The reason is the following commit, while simply ignores bridge
resources when enabling a pci device:

Commit 5c48b108 ("um: take arch/um/sys-x86 to arch/x86/um") broke the
make target update-po-config, as its symlink trick (again) fails.
(Previous breakage was fixed with commit bdc69ca4 ("kconfig: change
update-po-config to reflect new layout of arch/um").)
The new UML layout allows to drop the symlick trick entirely. And if,
one day, another architecture supports UML too, that should now work
without again breaking this make target.

Before registering an adapter to i2c subsystem, we need make sure
driver is ready for incoming i2c xfer, becasue the i2c_add_adapter()
may trigger a i2c device driver's proble function which may start
some real i2c xfer. I met this issue when integrating a TSC2007 i2c
touch screen device with the i2c-eg20t driver.
This patch will call request_irq() and hw init before calling
i2c_add_adapter().

Currently the fifo depth is set to zero for OMAP4 which disables
the FIFO usage. This patch enables the FIFO usage for I2C transactions
on OMAP4 also.

s3c24xx_i2c_parse_dt_gpio is called when cfg_gpio is not defined
in the platform data of the i2c device. When DT is not enabled,
the above function always returns -EINVAL. Since there can be
some i2c devices which don't need to configure any gpio lines,
the probe of such devices would fail here. Changing the default
return value to success would fix this issue.

Tests show that the original large intervals can easily make the dirty
limit exceeded on 100 concurrent dd's. So adapt to as large as the
next check point selected by the dirty throttling algorithm.

Some active adaptors (VGA usually) only have two lanes at 2.7GHz.
That's a maximum pixel clock of 144MHz at 8bpc, but 192MHz at 6bpc.
Fixes Asus UX31 panel being black at startup due to no valid modes since
dc22ee6fc1.
v2: Rebased to current code, resulting in the fix applying to EDP panels as
well. Also changed from spatio-temporal to just spatial dithering on
pre-ironlake, to be conssitent (and less visual flicker)

We were checking whether the supplied edid matched the connector it was
read from. We do this in case a DDC read returns an EDID for another
device on a multifunction or otherwise interesting card. However, we
failed to include LVDS as a digital device and so rejecting an otherwise
valid EDID.
Fixes the detection of the secondary SDVO LVDS panel on the Libretto
W105.

This prevents an in-kernel division by zero which happens when we are
asking for i915_chipset_val too quickly, or within a race condition
between the power monitoring thread and userspace accesses via debugfs.
The issue can be reproduced easily via the following command:
while ``; do cat /sys/kernel/debug/dri/0/i915_emon_status; done
This is particularly dangerous because it can be triggered by
a non-privileged user by just reading the debugfs entry.
This issue was also found independently by Konstantin Belousov
<kostikbel@gmail.com>, who proposed a similar patch.

All requests of a queue could be merged to other requests of other queue.
Such queue will not have request in it, but it's in service tree. This
will cause kernel oops.
I encounter a BUG_ON() in cfq_dispatch_request() with next patch, but the
issue should exist without the patch.

sched: Avoid SMT siblings in select_idle_sibling() if possible
stopped selecting an idle SMT sibling when there are no idle
cores in a single socket system.
Intent of the select_idle_sibling() was to fallback to an idle
SMT sibling, if it fails to identify an idle core. But this
fallback was not happening on systems where all the scheduler
domains had `SD_SHARE_PKG_RESOURCES' flag set.
Fix it. Slightly bigger patch of cleaning all these goto's etc
is queued up for the next release.

After commit 06222e491e (fs: handle
SEEK_HOLE/SEEK_DATA properly in all fs's that define their own llseek)
the behaviour of llseek() was changed so that it always revalidates
the file size. The bug appears to be due to a logic error in the
afore-mentioned commit, which always evaluates to 'true'.

The bisection implemented in unwind_find_origin() stopped to early. If
there is only a single entry left to check the original code just took
the end point as origin which might be wrong.

The btrfs io submission threads can build up massive plug lists. This
keeps things more reasonable so we don't hand over huge dumps of IO at
once.

per_cpu_ptr_to_phys() incorrectly rounds up its result for non-kmalloc
case to the page boundary, which is bogus for any non-page-aligned
address.
This affects the only in-tree user of this function - sysfs handler
for per-cpu 'crash_notes' physical address. The trouble is that the
crash_notes per-cpu variable is not page-aligned:
crash_notes = 0xc08e8ed4
PER-CPU OFFSET VALUES:

CPU 2: 37133000
CPU 3: 3713d000
So, the per-cpu addresses are:
crash_notes on CPU 0: f7a07ed4 => phys 36b57ed4

crash_notes on CPU 2: f7a1bed4 => phys 36b43ed4
crash_notes on CPU 3: f7a25ed4 => phys 36b39ed4
However, /sys/devices/system/cpu/cpu*/crash_notes says:

As you can see, all values are rounded down to a page
boundary. Consequently, this is where kexec sets up the NOTE segments,
and thus where the secondary kernel is looking for them. However, when
the first kernel crashes, it saves the notes to the unaligned
addresses, where they are not found.
Fix it by adding offset_in_page() to the translated page address.

While probing, fd sets up queue, probes hardware and tears down the
queue if probing fails. In the process, blk_drain_queue() kicks the
queue which failed to finish initialization and fd is unhappy about
that.

This fixes an odd bug found on a Dell PowerEdge 1850/0RC130
(BIOS A05 01/09/2006) where all of the modules doing pci_set_dma_mask
would fail with:
ata_piix 0000:00:1f.1: enabling device (0005 -> 0007)
ata_piix 0000:00:1f.1: can't derive routing for PCI INT A
ata_piix 0000:00:1f.1: BMDMA: failed to set dma mask, falling back to PIO
The issue was the Xen-SWIOTLB was allocated such as that the end of
buffer was stradling a page (and also above 4GB). The fix was
spotted by Kalev Leonid which was to piggyback on git commit
e79f86b2ef "swiotlb: Use page alignment
for early buffer allocation" which:
We could call free_bootmem_late() if swiotlb is not used, and
it will shrink to page alignment.
So alloc them with page alignment at first, to avoid lose two pages
And doing that fixes the outstanding issue.

d312ae878b "xen: use maximum reservation to limit amount of usable RAM"
clamped the total amount of RAM to the current maximum reservation. This is
correct for dom0 but is not correct for guest domains. In order to boot a guest
"pre-ballooned" (e.g. with memory=1G but maxmem=2G) in order to allow for
future memory expansion the guest must derive max_pfn from the e820 provided by
the toolstack and not the current maximum reservation (which can reflect only
the current maximum, not the guest lifetime max). The existing algorithm
already behaves this correctly if we do not artificially limit the maximum
number of pages for the guest case.
For a guest booted with maxmem=512, memory=128 this results in:

With this change "xl mem-set <domain> 512M" will successfully increase the

A user reported a problem booting into a new kernel with the old format inodes.
He was panicing in cow_file_range while writing out the inode cache. This is
because if the block group is not cached we'll just skip writing out the cache,
however if it gets dirtied again in the same transaction and it finished caching
we'd go ahead and write it out, but since we set cache_generation to the transid
we think we've already truncated it and will just carry on, running into
cow_file_range and blowing up. We need to make sure we only set
cache_generation if we've done the truncate. The user tested this patch and
verified that the panic no longer occured. Thanks,

I've been hitting this BUG_ON() in btrfs_orphan_add when running xfstest 269 in
a loop. This is because we will add an orphan item, do the truncate, the
truncate will fail for whatever reason (*cough*ENOSPC*cough*) and then we're
left with an orphan item still in the fs. Then we come back later to do another
truncate and it blows up because we already have an orphan item. This is ok so
just fix the BUG_ON() to only BUG() if ret is not EEXIST. Thanks,

We were occasionaly leaking space when running xfstest 269. This is because if
we failed to start the transaction in the truncate loop we'd just goto out, but
we need to break so that the inode is removed from the orphan list and the space

Running xfstests 269 with some tracing my scripts kept spitting out errors about
releasing bytes that we didn't actually have reserved. This took me down a huge
rabbit hole and it turns out the way we deal with reserved_extents is wrong,
we need to only be setting it if the reservation succeeds, otherwise the free()
method will come in and unreserve space that isn't actually reserved yet, which
can lead to other warnings and such. The math was all working out right in the
end, but it caused all sorts of other issues in addition to making my scripts
yell and scream and generally make it impossible for me to track down the
original issue I was looking for. The other problem is with our error handling
in the reservation code. There are two cases that we need to deal with
1) We raced with free. In this case free won't free anything because csum_bytes
is modified before we dro the lock in our reservation path, so free rightly
doesn't release any space because the reservation code may be depending on that
reservation. However if we fail, we need the reservation side to do the free at
that point since that space is no longer in use. So as it stands the code was
doing this fine and it worked out, except in case #2
2) We don't race with free. Nobody comes in and changes anything, and our
reservation fails. In this case we didn't reserve anything anyway and we just
need to clean up csum_bytes but not free anything. So we keep track of
csum_bytes before we drop the lock and if it hasn't changed we know we can just
decrement csum_bytes and carry on.
Because of the case where we can race with free()'s since we have to drop our
spin_lock to do the reservation, I'm going to serialize all reservations with
the i_mutex. We already get this for free in the heavy use paths, truncate and
file write all hold the i_mutex, just needed to add it to page_mkwrite and
various ioctl/balance things. With this patch my space leak scripts no longer
scream bloody murder. Thanks,

Now that we're properly keeping track of delayed inode space we've been getting
a lot of warnings out of btrfs_dirty_inode() when running xfstest 83. This is
because a bunch of people call mark_inode_dirty, which is void so we can't
return ENOSPC. This needs to be fixed in a few areas
1) file_update_time - this updates the mtime and such when writing to a file,
which will call mark_inode_dirty. So copy file_update_time into btrfs so we can
call btrfs_dirty_inode directly and return an error if we get one appropriately.
2) fix symlinks to use btrfs_setattr for ->setattr. For some reason we weren't
setting ->setattr for symlinks, even though we should have been. This catches
one of the cases where we were getting errors in mark_inode_dirty.
3) Fix btrfs_setattr and btrfs_setsize to call btrfs_dirty_inode directly
instead of mark_inode_dirty. This lets us return errors properly for truncate
and chown/anything related to setattr.
4) Add a new btrfs_fs_dirty_inode which will just call btrfs_dirty_inode and
print an error if we have one. The only remaining user we can't control for
this is touch_atime(), but we don't really want to keep people from walking
down the tree if we don't have space to save the atime update, so just complain
but don't worry about it.
With this patch xfstests 83 complains a handful of times instead of hundreds of

Al pointed out we have some random problems with the way we account for
num_workers_starting in the async thread stuff. First of all we need to make
sure to decrement num_workers_starting if we fail to start the worker, so make
__btrfs_start_workers do this. Also fix __btrfs_start_workers so that it
doesn't call btrfs_stop_workers(), there is no point in stopping everybody if we
failed to create a worker. Also check_pending_worker_creates needs to call
__btrfs_start_work in it's work function since it already increments
num_workers_starting.
People only start one worker at a time, so get rid of the num_workers argument
everywhere, and make btrfs_queue_worker a void since it will always succeed.

The Smack LSM hook for security_d_instantiate checks
the inode's i_op->getxattr value to determine if the
containing filesystem supports extended attributes.
The BTRFS filesystem sets the inode's i_op value only
after it has instantiated the inode. This results in
Smack incorrectly giving new BTRFS inodes attributes
from the filesystem defaults on the assumption that
values can't be stored on the filesystem. This patch
moves the assignment of inode operation vectors ahead
of the calls to d_instantiate, letting Smack know that
the filesystem supports extended attributes. There
should be no impact on the performance or behavior of
BTRFS.

Since we have the free space caches, btrfs_orphan_cleanup also runs for
the tree_root. Unfortunately this also cleans up the orphans used to mark
subvol deletions in progress.
Currently if a subvol deletion gets interrupted twice by umount/mount, the
deletion will not be continued and the space permanently lost, though it
would be possible to write a tool to recover those lost subvol deletions.
This patch checks if the orphan belongs to a subvol (dead root) and skips
the deletion.

When we use raid0 as the data profile, df command may show us a very
inaccurate value of the available space, which may be much less than the
real one. It may make the users puzzled. Fix it by changing the calculation
of the available space, and making it be more similar to a fake chunk
allocation.

Btrfsck report errors after the 83th case of xfstests was run, The error
number is 400, it means the used disk space of the file is wrong.
The reason of this bug is that:
The file truncation may fail when the space of the file system is not enough,
and leave some file extents, whose offset are beyond the end of the files.
When we want to expand those files, we will drop those file extents, and
put in dummy file extents, and then we should update the i-node. But btrfs
forgets to do it.
This patch adds the forgotten i-node update.

Btrfsck report error 100 after the 83th case of xfstests was run, it means
the i_size of the file is wrong.
The reason of this bug is that:
Btrfs increased i_size of the file at the beginning, but it failed to expand
the file, and failed to update the i_size to the old size because there is no
enough space in the file system, so we found a wrong i_size.
This patch fixes this bug by updating the i_size just when we pass the file
expanding and get enough space to update i-node.

Use DCB notifiers to set the skb priority to allow packets
to be steered and tagged correctly over DCB enabled drivers
that setup traffic classes.
This allows queue_mapping() routines to be removed in these
drivers that were previously inspecting the ethertype of
every skb to mark FCoE/FIP frames.

All our revision macros are defined with the entire
32-bits which we read from GSNPSID register, so we
must cache all 32-bits properly rather than masking
the top 16-bits.
This will fix all revision checks we have on current
driver.

Command cancel path cancels the current command and moves
it to free command queue. While doing that it deletes the
command entry from the pending list. This is not correct
as the entry has been already deleted from the pending
list at 'mwifiex_exec_next_cmd'. Fixing it.
Also making sure the stale command pointer is cleaned and
unaccessible for later use.

During test of one IB card with guest VM, found that, msi is not
initialized properly.
It turns out __write_msi_msg will do nothing if device current_state is
not PCI_D0. And, that pci device does not have pm_cap in guest VM.
There is an error in setting of power state to PCI_D0 in
pci_enable_device(), but error is not returned for this. Following is
code flow:

Here, from platform_pci_set_power_state(), acpi_pci_set_power_state() is
getting called and that is failing with ENODEV because of following
condition:

Because of that, pci_update_current_state() is not getting called.
With this patch, if device power state can not be set via
platform_pci_set_power_state and that device does not have native pm

During session recovery, the conn_stop call will trigger a flush
to all outstanding SCSI cmds in the xmit queue. This will set
all outstanding task->sc to NULL prior to the session_teardown
call which frees the task memory.
In the bnx2i SCSI response processing path, only the task was being checked
for NULL under the session lock before the task->sc->request dereferencing.
If there are outstanding SCSI cmd responses pending for process, the
following kernel panic can be exposed where task->sc was found to be NULL.

the nex_idx is not updated,

This means there is a risk that the 'idx < max_ddbs' condition will never
met and the loop will loop forever.
Fix this by explicitly increasing the next_idx in the error condition.

With open-iscsi support, target entries persisted in the FLASH were not
login. Added support in the qla4xxx driver to do the login on probe
time to the target entries saved in the FLASH by user.
With this changes upgrade to the new kernel with open-iscsi support in
qla4xxx will ensure users original target entries login on driver load

zfcp_scsi_slave_destroy erroneously always tried to finish its task
even if the corresponding previous zfcp_scsi_slave_alloc returned
early. This can lead to kernel page faults on accessing uninitialized
fields of struct zfcp_scsi_dev in zfcp_erp_lun_shutdown_wait. Take the
port field of the struct to determine if slave_alloc returned early.
This zfcp bug is exposed by 4e6c82b (in turn fixing f7c9c6b to be
compatible with 21208ae) which can call slave_destroy for a
corresponding previous slave_alloc that did not finish.

The label 'out_bdi' should be followed by bdi_destroy() instead of
fput() which should be after the 'out_fput' label.
If bdi_setup_and_register() fails then jump to the 'out_fput' label
instead of the 'out_bdi' one.
If fget(data.info_fd) fails then jump to the previously fixed 'out_bdi'
label to call bdi_destroy() otherwise the bdi object will not be
destroyed.

If a page has been read into memory and never been written, it has no
buffers, but we should handle the page in truncate or punch hole.
VFS code of writing operations has handled holes correctly, so this
patch removes the code handling holes in writing operations.

If there is an unwritten but clean buffer in a page and there is a
dirty buffer after the buffer, then mpage_submit_io does not write the
dirty buffer out. As a result, da_writepages loops forever.
This patch fixes the problem by checking dirty flag.

If the pte mapping in generic_perform_write() is unmapped between
iov_iter_fault_in_readable() and iov_iter_copy_from_user_atomic(), the
"copied" parameter to ->end_write can be zero. ext4 couldn't cope with
it with delayed allocations enabled. This skips the i_disksize
enlargement logic if copied is zero and no new data was appeneded to
the inode.

This is 100% reproducible with some autonuma development code tuned in
a very aggressive manner (not normal way even for knumad) which does
"exotic" changes to the ptes. It wouldn't normally trigger but I don't
see why it can't happen normally if the page is added to swap cache in
between the two faults leading to "copied" being zero (which then
hangs in ext4). So it should be fixed. Especially possible with lumpy
reclaim (albeit disabled if compaction is enabled) as that would
ignore the young bits in the ptes.

Given that dm timer framework doesn't support request of clocks
by soft | hard irqs because some recent changes, tidspbridge needs
to request its clocks on init and enable/disable them on demand.
This was first seen on 3.2-rc1.

After commit 8e2ec63917 ("ipv6: don't
use inetpeer to store metrics for routes.") the test in rt6_alloc_cow()
for setting the ANYCAST flag is now wrong.
'rt' will always now have a plen of 128, because it is set explicitly
to 128 by ip6_rt_copy.
So to restore the semantics of the test, check the destination prefix
length of 'ort'.

Enable isp1704 power on/off) added power management for isp1704.
However, the transceiver should be powered on by default,
otherwise USB doesn't work at all for networking during
boot.

The m41t80 driver can read and set the alarm, but it doesn't
seem to have a functional alarm irq.
This causes failures when the generic core sees alarm functions,
but then cannot use them properly for things like UIE mode.
Disabling the alarm functions allows proper error reporting,
and possible fallback to emulated modes. Once someone fixes
the alarm irq functionality, this can be restored.

If the alarm time programming in the rtc is ever in the past, it won't fire,
and any other alarm will be queued after it so they won't fire either.
So any time that the alarm might be in the past, we need to trigger
the irq handler to ensure the old alarm is cleared and the timer queue
is fully in the future.
This can happen:
- when we first initialise the alarm
- when we set the time in the rtc.
so follow both of these by scheduling the timer work function.

Commit 0d52f54e2e (PCI / ACPI: Make
acpiphp ignore root bridges using PCIe native hotplug) added code
that made the acpiphp driver completely ignore PCIe root complexes
for which the kernel had been granted control of the native PCIe
hotplug feature by the BIOS through _OSC. Unfortunately, however,
this was a mistake, because on some systems there were PCI bridges
supporting PCI (non-PCIe) hotplug under such root complexes and
those bridges should have been handled by acpiphp.
For this reason, revert the changes made by the commit mentioned
above and make register_slot() in drivers/pci/hotplug/acpiphp_glue.c
avoid registering hotplug slots for PCIe ports that belong to
root complexes with native PCIe hotplug enabled (which means that
the BIOS has granted the kernel control of this feature for the
given root complex). This is reported to address the original
issue fixed by commit 0d52f54e2e and
to work on the system where that commit broke things.

Huawei use the product code HUAWEI_PRODUCT_E353 (0x1506) for a
number of different devices, which each can appear with a number
of different descriptor sets. Different types of interfaces
can be identified by looking at the subclass and protocol fields
Subclass 1 protocol 8 is actually the data interface of a CDC
ECM set, with subclass 1 protocol 9 as the control interface.
Neither support serial data communcation, and cannot therefore
be supported by this driver.
At the same time, add a few other sets which appear if the
device is configured in "Windows mode" using this modeswitch
message:

Commit 06222e491e got the if wrong so that
it always evaluates as true. This is semantically harmless, but makes
SEEK_CUR and SEEK_SET needlessly query the server.
Rewrite the if to explicitly enumerate the cases we DO need a valid i_size
to make this code less fragile.

Found one system with UEFI/iBFT, kernel does not detect the iBFT during
iscsi_ibft module loading.
Root cause: on x86 (UEFI), we are calling of find_ibft_region() much earlier
- specifically in setup_arch() before ACPI is enabled.
Try to split acpi checking code out and call that later
At that time ACPI iBFT already get permanent mapped with ioremap.
So isa_virt_to_bus() will get wrong phys from right virt address.
We could just skip that phys address printing.
For legacy one, print the found address early.

The test in fuse_file_llseek() "not SEEK_CUR or not SEEK_SET" always evaluates
to true.
This was introduced in 3.1 by commit 06222e49 (fs: handle SEEK_HOLE/SEEK_DATA
properly in all fs's that define their own llseek) and changed the behavior of
SEEK_CUR and SEEK_SET to always retrieve the file attributes. This is a
performance regression.
Fix the test so that it makes sense.

- retrieving more than one page would yield repeated instances of the
first page
- if more than FUSE_MAX_PAGES_PER_REQ pages were requested than the
request page array would overflow
fuse_retrieve() was added in 2.6.36 and these bugs had been there since the
beginning.

Exactly like roundup_pow_of_two(1), the rounddown version was buggy for
the case of a compile-time constant '1' argument. Probably because it
originated from the same code, sharing history with the roundup version
from before the bugfix (for that one, see commit 1a06a52ee1: "Fix
roundup_pow_of_two(1)").
However, unlike the roundup version, the fix for rounddown is to just
remove the broken special case entirely. It's simply not needed - the
generic code
1UL << ilog2(n)
does the right thing for the constant '1' argment too. The only reason
roundup needed that special case was because rounding up does so by
subtracting one from the argument (and then adding one to the result)
causing the obvious problems with "ilog2(0)".
But rounddown doesn't do any of that, since ilog2() naturally truncates
(ie "rounds down") to the right rounded down value. And without the
ilog2(0) case, there's no reason for the special case that had the wrong
value.
tl;dr: rounddown_pow_of_two(1) should be 1, not 0.

gred_change_vq() is called under sch_tree_lock(sch).
This means a spinlock is held, and we are not allowed to sleep in this
context.
We might pre-allocate memory using GFP_KERNEL before taking spinlock,
but this is not suitable for stable material.

Same fix as 731abb9cb2 for ipip and sit tunnel.
Commit 1c5cae815d removed an explicit call to dev_alloc_name in
ipip_tunnel_locate and ipip6_tunnel_locate, because register_netdevice
will now create a valid name, however the tunnel keeps a copy of the
name in the private parms structure. Fix this by copying the name back
after register_netdevice has successfully returned.
This shows up if you do a simple tunnel add, followed by a tunnel show:

There is no obvious reason to add a default multicast route for loopback
devices, otherwise there would be a route entry whose dst.error set to
-ENETUNREACH that would blocking all multicast packets.

The problem is that the resulting routing table depends on the sequence
of interface's initialization and in some situation, that would block all
muticast packets. Suppose there are two interfaces on my computer
(lo and eth0), if we initailize 'lo' before 'eth0', the resuting routing
table(for multicast) would be

When sending multicasting packets, routing subsystem will return the first
route entry which with a error set to -101(ENETUNREACH).
I know the kernel will set the default ipv6 address for 'lo' when it is up
and won't set the default multicast route for it, but there is no reason to
stop 'init' program from setting address for 'lo', and that is exactly what
systemd did.
I am sure there is something wrong with kernel or systemd, currently I preferred
kernel caused this problem.

The stations always chooses 1Mbps for all trasmitting frames,
whenever the AP is configured to lock the supported rates.
As the max phy rate is always set with the 4th from highest phy rate,
this assumption might be wrong if we have less than that. Fix that.

wait_for_completion_interruptible_timeout() returns -ERESTARTSYS if
interrupted so completion_rc needs to be signed. The current code
probably returns -ETIMEDOUT if we hit this situation, but after this
patch is applied it will return -ERESTARTSYS.

Check the IEEE80211_TX_CTL_ASSIGN_SEQ flag from mac80211, then decide how to
set the TX_CMD_FLG_SEQ_CTL_MSK bit. Setting the wrong bit in BAR frame whill
make the firmware to increment the sequence number which is incorrect and
cause unknown behavior.

From a talk with him at #linuxtv irc, he strong feeling that some apps
like MythTV will do a lot a lot of I2C I/O and now it adds locks,
with utimatelly means more delays. This could cause bad effects.
There is a new patch for 3.3 that re-writes af9013 in order to limit
I2C I/O. and thus those patches could be nice to have together
because those user who has has problems are most likely MythTV users.
So, let's revert this commit for now.

This is due to getting ext_depth(inode) == 0x101 and therefore running
off the end of the path array in ext4_ext_drop_refs into following
unallocated structures.
This fixes it by adding the necessary le16_to_cpu.

The old PowerMac swim3 driver has some "interesting" locking issues,
using a private lock and failing to lock the queue before completing
requests, which triggered WARN_ONs among others.
This rips out the private lock, makes everything operate under the
block queue lock, and generally makes things simpler.
We used to also share a queue between the two possible instances which
was problematic since we might pick the wrong controller in some cases,
so make the queue and the current request per-instance and use
queuedata to point to our private data which is a lot cleaner.
We still share the queue lock but then, it's nearly impossible to actually
use 2 swim3's simultaneously: one would need to have a Wallstreet
PowerBook, the only machine afaik with two of these on the motherboard,
and populate both hotswap bays with a floppy drive (the machine ships
only with one), so nobody cares...
While at it, add a little fix to clear up stale interrupts when loading
the driver or plugging a floppy drive in a bay.

This function can wait for 5min under certain scenarios. One of them is when
the port is down from switch and bus reset is issued. The bus reset used to
wait for 5 minutes for the loop and upper layer callers used to hang and give
stack trace because of getting stuck for 120 sec. It is legacy code that was
used when the driver used to do queuing of the commands.

Synaptics touchpads on several Dell laptops, particularly Vostro V13
systems, may not respond properly to PS/2 commands and queries immediately
after resuming from suspend to RAM. This leads to unresponsive touchpad
after suspend/resume cycle.
Adding a 1-second delay after resetting the device allows touchpad to
finish initializing (calibrating?) and start reacting properly.

The ati_remote driver currently uses 2-byte scancodes. However, one of
those bytes is actually a checksum and therefore shouldn't be considered
as part of the scancode.
Fix the driver to only use the actual data byte as a scancode and to
check the checksum itself. Update the bundled keymaps accordingly.
Since ati_remote was only migrated to the rc subsystem for 3.2, the
previous scancodes weren't emitted on any stable kernel.

On 64-bit platforms assigning a pointer to a 32-bit variable causes a
compiler warning and cannot actually work. Soc-camera currently doesn't
support any 64-bit systems, but such platforms can be added in the
and in any case compiler warnings should be avoided.

mmc_suspend_host() tries to claim host during suspend
and release it only when the bus suspend operation is
compeleted. If CONFIG_MMC_UNSAFE_RESUME is defined and
the host is flagged as removable, mmc_suspend_host()
tries to remove the card. In this process, the file system
sync can get blocked trying to acquire host which is already
claimed by mmc_suspend_host() causing deadlock.
Fix this deadlock by releasing host before ->remove() is called.

The eMMC 4.5 devices respond to only RESET and AWAKE command in the
sleep state. Hence the mmc switch command to notify power off state
should be sent before the device enters sleep state.
This patch fixes the same.

Adds a quirk that sets the data read timeout to a fixed value instead
of relying on the information in the CSD. The timeout value chosen
is 300ms since that has proven enough for the problematic cards found,
but could be increased if other cards require this.
This patch also enables this quirk for certain Micron cards known to
have this problem.

When we can't configure the dma channel we want to fall
back to PIO. We do this by setting host->do_dma to zero.
This does not work as do_dma is used to see whether dma
can be used for the current transfer. Instead, we have
to set host->dma to NULL.

efi_call_phys_prelog() sets up a 1:1 mapping of the physical address
range in swapper_pg_dir. Instead of replacing then restoring entries
in swapper_pg_dir we should be using initial_page_table which already
contains the 1:1 mapping.
It's safe to blindly switch back to swapper_pg_dir in the epilog
because the physical EFI routines are only called before
efi_enter_virtual_mode(), e.g. before any user processes have been
forked. Therefore, we don't need to track which pgd was in %cr3 when
we entered the prelog.
The previous code actually contained a bug because it assumed that the
kernel was loaded at a physical address within the first 8MB of ram,
usually at 0x100000. However, this isn't the case with a
CONFIG_RELOCATABLE=y kernel which could have been loaded anywhere in
the physical address space.
Also delete the ancient (and bogus) comments about the page table
being restored after the lock is released. There is no locking.

btrfs_end_bio checks the number of errors on a bio against the max
number of errors allowed before sending any EIOs up to the higher
levels.
If we got enough copies of the bio done for a given raid level, it is
supposed to clear the bio error flag and return success.
We have pointers to the original bio sent down by the higher layers and
pointers to any cloned bios we made for raid purposes. If the original
bio happens to be the one that got an io error, but not the last one to
finish, it might not have the BIO_UPTODATE bit set.
Then, when the last bio does finish, we'll call bio_end_io on the
original bio. It won't have the uptodate bit set and we'll end up
sending EIO to the higher layers.
We already had a check for this, it just was conditional on getting the
IO error on the very last bio. Make the check unconditional so we eat
the EIOs properly.

Since commit 6571534 (plat-mxc: iomux-v3.h: implicitly enable
pull-up/down when that's desired) was in, the power button on imx51
babbage board stopped working because it's pulled up by mistake.
The patch removes the pull-up setting from the pad configuration for
that gpio to make the power button back to work.

ssb: fix init regression of hostmode PCI core
In ssb_pcicore_fix_sprom_core_index() the sprom on the PCI core is
accessed, but the sprom only exists when the ssb bus is connected over
a PCI bus to the rest of the system and not when the SSB Bus is the
main system bus. SoCs sometimes have a PCI host controller and there
this code will not be executed, but there are some old SoCs with an PCI
controller in client mode around and ssb_pcicore_fix_sprom_core_index()
should not be called on these devices too. The PCI controller on these
devices are unused, but without this fix it results in an Data bus
error when it gets initialized.

Some trace shows lots of bdi_dirty=0 lines where it's actually some
small value if w/o the accounting errors in the per-cpu bdi stats.
In this case the max pause time should really be set to the smallest
(non-zero) value to avoid IO queue underrun and improve throughput.

On a system with 1 local mount and 1 NFS mount, if the NFS server
becomes not responding when dd to the NFS mount, the NFS dirty pages may
exceed the global dirty limit and _every_ task involving writing will be
blocked. The whole system appears unresponsive.
The workaround is to permit through the bdi's that only has a small
number of dirty pages. The number chosen (bdi_stat_error pages) is not
enough to enable the local disk to run in optimal throughput, however is
enough to make the system responsive on a broken NFS mount. The user can
then kill the dirtiers on the NFS mount and increase the global dirty
limit to bring up the local disk's throughput.
It risks allowing dirty pages to grow much larger than the global dirty
limit when there are 1000+ mounts, however that's very unlikely to happen,
especially in low memory profiles.

We do "floating proportions" to let active devices to grow its target
share of dirty pages and stalled/inactive devices to decrease its target
share over time.
It works well except in the case of "an inactive disk suddenly goes
busy", where the initial target share may be too small. To mitigate
this, bdi_position_ratio() has the below line to raise a small
bdi_thresh when it's safe to do so, so that the disk be feed with enough
dirty pages for efficient IO and in turn fast rampup of bdi_thresh:
bdi_thresh = max(bdi_thresh, (limit - dirty) / 8);
balance_dirty_pages() normally does negative feedback control which
adjusts ratelimit to balance the bdi dirty pages around the target.
In some extreme cases when that is not enough, it will have to block
the tasks completely until the bdi dirty pages drop below bdi_thresh.

The SMDK board uses LT3591 as backlight LED driver of LTE480WV LCD.
According to the LT3591 datasheet, the switching frequency should
be 1MHz. So, PWM period is calculated by following formula:
PWM period = 1/switching frequency
= 1/1MHz
= 1000ns
Thus, the PWM backlight period should be 1000ns.

Emmanuel noticed that when mac80211 stops the queues
for aggregation that can leave a packet pending. This
packet will be given to the driver after the AMPDU
callback, but as a non-aggregated packet which messes
up the sequence number etc.
I also noticed by looking at the code that if packets
are being processed while we clear the WANT_START bit,
they might see it cleared already and queue up on
tid_tx->pending. If the driver then rejects the new
aggregation session we leak the packet.
Fix both of these issues by changing this code to not
stop the queues at all. Instead, let packets queue up
on the tid_tx->pending queue instead of letting them
get to the driver, and add code to recover properly
in case the driver rejects the session.
(The patch looks large because it has to move two
functions to before their new use.)

The code for setting the address of the internal TBI PHY was
convoluted enough without a maze of ifdefs. Clean it up a bit
so we allow the logic to fail down to -ENODEV at the end of
the if/else ladder, rather than using ifdefs to repeat the same
failure code over and over.
Also, remove the support for the auto-configuration. I'm not aware of
anyone using it, and it ends up using the bus mutex before it's been
initialized.

We have been using i_lock to protect all kinds of data structures in the
ceph_inode_info struct, including lists of inodes that we need to iterate
over while avoiding races with inode destruction. That requires grabbing
a reference to the inode with the list lock protected, but igrab() now
takes i_lock to check the inode flags.
Changing the list lock ordering would be a painful process.
However, using a ceph-specific i_ceph_lock in the ceph inode instead of
i_lock is a simple mechanical change and avoids the ordering constraints
imposed by igrab().

This doesn't interact with resizing well, since it doesn't set the
size of the device to the size at the snapshot. It's also an expensive
operation to be synchronous. Rollback can still be done with the
userspace rbd tool.

The pdev->id is used in several places for different purpose. All
these uses assume it's always the id of fec device which is >= 0.
However this is only true for non-DT case. When DT plays, pdev->id
is always -1, which will break these pdev->id users.
Instead of fixing all these users one by one, this patch introduces
a new member 'dev_id' to 'struct fec_enet_private' for holding the
correct fec device id, and replaces all the existing uses of pdev->id
with this dev_id.

When receiving a DEL change for a client due to a roaming event (change is
marked with TT_CLIENT_ROAM), each node has to check if the client roamed
to itself or somewhere else.
In the latter case the global entry is kept to avoid having no route at all
otherwise we can safely delete the global entry

In case of a client roaming from node A to node B, the latter have to mark the
corresponding global entry with TT_CLIENT_ROAM (instead of TT_CLIENT_PENDING).
Marking a global entry with TT_CLIENT_PENDING will end up in keeping such entry
forever (because this flag is only meant to be used with local entries and it is
never checked on global ones).
In the worst case (all the clients roaming to the same node A) the local and the
global table will contain exactly the same clients. Batman-adv will continue to
work, but the memory usage is duplicated.

Commit cfcde11c3d ("IB/mlx4: Use flow counters on IBoE ports") added
code that sets elements of counters[] to -1 if no counter is allocated,
but then goes ahead and passes every entry to mlx4_counter_free() on
shutdown. This is a bad idea, especially if MLX4_DEV_CAP_FLAG_COUNTERS
isn't set so there isn't even an underlying bitmap to free from.

In the unlikely case that a platform registers a PMU platform_device
when running on a CPU that is unsupported by perf, we will encounter a
NULL dereference when trying to assign the platform_device to the
cpu_pmu structure.
This patch checks that the CPU is supported by perf before assigning
the platform_device.

The linker places the unwind tables in readonly sections. So when using
an XIP kernel these are located in ROM and cannot be modified.
For that reason the current approach to convert the relative offsets in
the unwind index to absolute addresses early in the boot process doesn't
work with XIP.
The offsets in the unwind index section are signed 31 bit numbers and
the structs are sorted by this offset. So it first has offsets between
0x40000000 and 0x7fffffff (i.e. the negative offsets) and then offsets
between 0x00000000 and 0x3fffffff. When seperating these two blocks the
numbers are sorted even when interpreting the offsets as unsigned longs.
So determine the first non-negative entry once and track that using the
new origin pointer. The actual bisection can then use a plain unsigned
long comparison. The only thing that makes the new bisection more
complicated is that the offsets are relative to their position in the
index section, so the key to search needs to be adapted accordingly in
each step.
Moreover several consts are added to catch future writes and rename the
member "addr" of struct unwind_idx to "addr_offset" to better match the
new semantic. (This has the additional benefit of breaking eventual
users at compile time to make them aware of the change.)
In my tests the new algorithm was a tad faster than the original and has
the additional upside of not needing the initial conversion and so saves
some boot time and it's possible to unwind even earlier.

Commit 1b9f95f8ad (ARM: prepare for removal of a bunch of <mach/memory.h>
files) introduced CONFIG_PHYS_OFFSET but the Kconfig hex prompt did not
provide a default value.
This has the undesired side effect of breaking a reportedly used
trick for updating defconfigs on the fly for routine buildtesting
across all arch and all platforms, i.e.
cp /path/to/somedefconfig .config ; yes "" | make oldconfig
because the config system will endlessly loop until a valid address is
provided.
However we can't just pick a random default value since it is likely to
be wrong for the majority of the boards as the right answer for this
option is quite varied. So the fact that the config system insists on
having a proper value be entered is actually a good thing.
It turns out that only at91x40_defconfig has this problem because it has
CONFIG_MMU=n. However, in the !MMU case, there is already a CONFIG_DRAM_BASE
value that can be used here. So let's use that as a default in that case
and suppress the redundant CONFIG_PHYS_OFFSET prompt.
Eventually the DRAM_BASE config option could simply be replaced by
PHYS_OFFSET directly, but that's a larger change better suited for later.

Currently SND_SOC_JZ4740_CODEC depends on SOC_JZ4740 but SOC_JZ4740 is not
defined in any Kconfig. Thus the codec driver will not be built when select
"Build all ASoC CODEC drivers".
(Unless it is selected by SND_JZ4740_SOC_QI_LB60).
Remove the dependency with SOC_JZ4740, then this code driver can be built when
select "Build all ASoC CODEC drivers".

All the PCI BARs of a device are enabled when the device is enabled
using pci_enable_device(). This unnecessarily enables SRIOV BARs of the
device.
On some platforms, which do not support SRIOV as yet, the
pci_enable_device() fails to enable the device if its SRIOV BARs are not
allocated resources correctly.
The following patch fixes the above problem. The SRIOV BARs are now
enabled when IOV capability of the device is enabled in sriov_enable().
NOTE: Note, there is subtle change in the pci_enable_device() API. Any
driver that depends on SRIOV BARS to be enabled in pci_enable_device()
can fail.
The patch has been touch tested on power and x86 platform.

cfq_cic_link() has race condition. When some processes which shared ioc
issue I/O to same block device simultaneously, cfq_cic_link() returns -EEXIST
sometimes. The race condition might stop I/O by following steps:
step 1: Process A: Issue an I/O to /dev/sda
step 2: Process A: Get an ioc (iocA here) in get_io_context() which does not
linked with a cic for the device
step 3: Process A: Get a new cic for the device (cicA here) in
cfq_alloc_io_context()
step 4: Process B: Issue an I/O to /dev/sda
step 5: Process B: Get iocA in get_io_context() since process A and B share the
same ioc
step 6: Process B: Get a new cic for the device (cicB here) in
cfq_alloc_io_context() since iocA has not been linked with a
cic for the device yet
step 7: Process A: Link cicA to iocA in cfq_cic_link()
step 8: Process A: Dispatch I/O to driver and finish it
step 9: Process B: Try to link cicB to iocA in cfq_cic_link()
But it fails with showing "cfq: cic link failed!" kernel
message, since iocA has already linked with cicA at step 7.
step 10: Process B: Wait for finishig I/O in get_request_wait()
The function does not wake up, when there is no I/O to the
device.
When cfq_cic_link() returns -EEXIST, it means ioc has already linked with cic.
So when cfq_cic_link() return -EEXIST, retry cfq_cic_lookup().

Currently write(2) to a file is not interruptible by any signal.
Sometimes this is desirable, e.g. when you want to quickly kill a
process hogging your disk. Also, with commit 499d05ecf9 ("mm: Make
task in balance_dirty_pages() killable"), it's necessary to abort the
current write accordingly to avoid it quickly dirtying lots more pages
at unthrottled rate.
This patch makes write interruptible by SIGKILL. We do not allow write
to be interruptible by any other signal because that has larger
potential of screwing some badly written applications.

If we handled an error condition, then nfs4_recovery_handle_error should
return '0' so that the state recovery thread can continue.
Also ensure that nfs4_check_lease() continues to abort if we haven't got
any credentials by having it return ENOKEY (which is not handled).

We weren't filling in the transfer length of the
flush cache command (it transfers 4 bytes of zeroes).
Firmware didn't seem to be bothered by this, but it

In case pixel format is modified in set_fmt by the driver,
the changes are not propagated back to the caller. Fix this
by adjusting passed data for TRY and ACTIVE format.
Also remove redundant pixel format code information from
struct m5mols_info, it's already available in 'ffmt' array.
Remove pad number validation in set/get_fmt, this is already
done in the core.

With 16-bit RGB565 colour format pixels are stored by the device in memory
in the following order:

This corresponds to V4L2_PIX_FMT_RGB565 fourcc, not V4L2_PIX_FMT_RGB565X.
This change is required to avoid trouble when setting up video pipeline
with the s5p-tv devices, so the colour formats at both devices can be
properly matched.

When a sensor with MIPI-CSI interface is attached through platform
data definition and the MIPI-CSI receiver is not selected in kernel
configuration s5p-fimc driver probe() will now succeed, issuing only
a warning. It was done this way to allow the driver to work even if
system configuration is not exactly right.
Instead make the driver's probe() fail if a MIPI-CSI sensor was
requested but s5p-csis module is not present.

Minimum vertical pixel size alignment for input and output DMA and
the scaler depend on color format, rotation, the IP instance and revision.
Make vertical pixel size of format and crop better fit for each SoC
revision and the IP instance by adding min_vsize_align attribute to
the FIMC variant data structure. It's now common for the DMA engines
and the scaler.

The "s5p-fimc-md" platform device platform_data is used to pass
attached camera sensor data. Not allowing device probe() to succeed
when it's null prevents using FIMC as a mem-to-mem device only.
Fix this by removing the platform_data check against null and
registering sensors only if platform_data is specified.
Also add logging of the information which /dev/video is assigned
to which device during probe().

When requested more than 2 buffers the buffer dequeue order was wrong
due to erroneous updating FIMC registers in every interrupt handler
call. This also fixes regression of resetting the output DMA buffer
pointer at wrong time, when some buffers are already queued in hardware.
The hardware is reset in the start_streaming callback in order to align
the H/W state with the software output buffer pointer (buf_index).
Additionally a simple write to S5P_CISCCTRL register is replaced with
a read/modification/write to make sure the scaler is not being disabled
in fimc_hw_set_scaler().

ST_LPM bit must not be initially set, so the first resume helper
call properly quiesce the device's operation.
Also fimc_runtime_suspend() at device remove is unneeded and
leads to unbalanced clock disable so remove it.

After i2c_unregister_device() has been called the client object can already
be freed and thus using the client pointer may lead to dereferencing freed
memory. Avoid this by saving the adapter pointer for further use before
i2c_unregister_device() call.

AF9015 firmware does not like if it gets interrupted by I2C adapter
request on some critical phases. During normal operation I2C adapter
is used only 2nd demodulator and tuner on dual tuner devices.
Override demodulator callbacks and use mutex for limit access to
those "critical" paths to keep AF9015 happy.

Keep the ZB clock enabled on sh73a0 to allow the BSC
to access external peripherals hooked up to CS signals.
This is needed to unbreak Ethernet support on sh73a0 boards
such as AG5EVM and Kota2 together with the following patch:

board make use of the ARM GIC hardware block as main interrupt
controller. The following commit changed the default behaviour
for non-device tree platforms and broke AG5EVM irq support:

This patch updates the Kota2 board support code to
use the recently merged TPU LED driver whenever
possible.
The sh73a0 SoC has 5 TPU hardware blocks each with
4 timer channels which in theory allows a total of
20 LEDs to be controlled by "leds-renesas-tpu"
driver instances. The Kota2 board has 4 LEDs connected
to GPIO pins that also come with TPU pin functions, so
this patch ties up these 4 LEDS and leaves the remaining
3 LEDS for the GPIO based LED driver.

Support PINT on sh73a0 and Kota2 using INTC PINT macros.
With this patch applied the Kota2 ethernet is handled
through one of the chained sh73a0 PINT interrupt controllers.
sh73a0 PINT support is included in 3.2-rc but the Kota2 board
code does not make use of this shared code base without this
patch. Without this patch kota2 on-board ethernet is broken.

For i945 and earlier chips, the backlight frequency value had the low
bit (of 16) fixed to zero. The Pineview code path handled this by just
exposing the backlight range as 15 bits while other chips had the
backlight range limited to 0 .. 0xfffe.
This patch makes everyone take the pineview code path, providing 15
bits of backlight duty cycle range which seems more than sufficient to
me.
Daniel Mack reported that writing 1 to bit 0 of the duty cycle
register was causing problems on his Samsung X20 notebook, even when
the duty cycle value was less than the maximum backlight value. (He
tried a value of 29749 with max_brightness of 29750). This patch never
writes a '1' to that bit.

The Ivybridge eDP control register looks like a cross between a
Cougarpoint PCH DP control register and a Sandybridge eDP control
register.
Where things trivially match, share the code. Where there are any
tricky bits, just split things out into two obviously separate code paths.

On IVB C0+ with newer BIOSes, the forcewake handshake has changed. There's
now a bitfield for different driver components to keep the GT powered
on. On Linux, we centralize forcewake handling in one place, so we
still just need a single bit, but we need to use the new registers if MT
forcewake is enabled.
This needs testing on affected machines. Please reply with your
tested-by if you had problems after a BIOS upgrade and this patch fixes
them.

struct request_queue is allocated with __GFP_ZERO so its "node" field is
zero before initialization. This causes an oops if node 0 is offline in
the page allocator because its zonelists are not initialized. From Dave
Young's dmesg:

and __alloc_pages_nodemask+0xb5 translates to a NULL pointer on
zonelist->_zonerefs.
The fix is to initialize q->node at the time of allocation so the correct
node is passed to the slab allocator later.
Since blk_init_allocated_queue_node() is no longer needed, merge it with
blk_init_allocated_queue().

this call may cleanup our module before it returns.
Gladly, the kthread API provides a simple wrapper for us. So lets use
module_put_and_exit() to avoid a race condition with the module cleanup code.
