Johan Hovold 526103b7a6 phy: qcom-qmp-combo: clean up DP callback names ...

Clean up and unify the DP callbacks by dropping the redundant "qcom" and
"phy" prefix and infix and by using a common naming scheme ("qmp" +
version + callback name).
